  4. Offer is strictly on a first-come, first-served basis while stocks last.
  5. Surcharge(s) apply for upgrades.
  6. Tests include blood glucose, lipid profile, uric acid, blood pressure, body mass index and medical review.
  7. Present and pay with a valid UOB Credit or Debit Card at any Health Lane Family Pharmacy outlet to redeem.
  8. Limited to one (1) offer per card, per visit, per transaction. Split bills are strictly not allowed.
  9. Partial redemption is not allowed.
  10. In the event of any inconsistencies between these terms and conditions and any promotional materials relating to this promotion, Health Lane Family Pharmacy’s terms and conditions shall prevail.
  11. All prices will be/may be subject to service charge, delivery charges and prevailing government taxes.
  12. Debit card that is linked to Islamic current/savings account shall be subject to its own terms and conditions for the purchase of Shariah-compliant goods and services only.
  13. Offer cannot be exchanged for cash or used in conjunction with other promotions, discounts, offer coupons, vouchers, loyalty programs and member privilege cards.
