Principal Cardmember: Age 21 or older
Supplementary Cardmember: Age 18 or older

  • Photocopy of MyKad (both sides)
  • Latest 3 months’ salary slip 1

AND any of the following

  • Latest Form B/BE (supported with tax payment receipt)

Self-employed

  • Photocopy of MyKad (both sides)
  • Photocopy of Business Registration Form
  • Latest BE Form with tax receipt
  • Latest 6 months’ bank statement (showing business turnover)
